The General Complaint Form may be used to report a complaint against Healthier Frederick’s Executive Director, staff member(s), Workgroup volunteer(s), and/or board member(s) and is used in conjunction with our Grievance Procedures. It is not intended for whistleblowing purposes.

Whistleblower Policy and Whistleblower Form

Healthier Frederick is committed to maintaining integrity, accountability, and transparency in all of its operations. The Whistleblower Form is intended to provide a way for individuals to report concerns about suspected fraud or financial misconduct, misuse of organizational resources, retaliation, and/or other ethical problems.

If your concern involves discrimination, bullying, sexual harassment, harassment, or unfair treatment, please refer to Healthier Frederick's Grievance Procedures and General Complaint Form above.

In the instance that a person does not feel comfortable submitting their report via the form to the Board President and Executive Director, they may download and complete the PDF form below and send it to vicepresident@healthierfrederick.org, which is monitored only by the Board Vice President.
