The Community Health Needs Assessment (CHNA) examines all the factors influencing our community’s health and seeks to provide a pathway towards better health.

Compares health statistics to the state and other benchmarks
Identifies available resources to address health issues and resident perceptions about health and social concerns
Outlines major health problems and health disparities
With significant input from the public, narrows health issues into a manageable set of priorities
The CHNA answers the following questions:
What are the major causes of illness and death in the community?
What health issues and behaviors are most concerning to local citizens and community organizations serving Frederick County?
What barriers and resources exist for residents to achieve better health?
The CHNA is sponsored by the Frederick County Health Department (FCHD) and Frederick Health (FH). Participation in the CHNA process by FCHD and FH fulfills regulatory and accreditation requirements for conducting a periodic community health assessment with public input and participation.
How was the 2025 CHNA created?
The 2025 CHNA was created using a collaborative, data and community-informed framework from the National Association of County and City Health Officials (NACCHO) called Mobilizing for Action through Planning and Partnerships (MAPP) 2.0 that brought together local organizations, experts, and community members to better understand the health needs and priorities of Frederick County residents.
Our CHNA Steering Committee and Data and Assessment Team were open to the public and provided leadership and direction for the assessment, ensuring alignment with our community’s needs and the MAPP 2.0 framework.
Primary data sources included a comprehensive Community Health Survey, 5 focus groups, and 5 Root Causes Surveys. Secondary data included over 40 health indicators complied from a range of state and federal sources, covering categories such as length of life, physical environment, and socioeconomic factors.
The Community Health Survey received 2,441 responses. Focus groups composed of:
Residents who identify as LGBTQIA+
Residents who identify as “often” or “always” lonely or isolated and have household income of $75,000 or less, with diverse race and ethnicity
Residents who are parents/guardians of a child under the age of 18 and have a place to live today but are worried about losing it in the future
Residents who identify as Hispanic or Latino/a who speak English as a second language and are age 30 or younger
Residents who identify as Black or African American.
The updated MAPP 2.0 framework placed greater emphasis on identifying root causes of health issues. This process also prioritized community voice, and as a result, the 2025 CHNA included more community input than ever before to ensure the findings reflected the true needs, lived experiences, and priorities of our community!
Current Priorities
These priorities were selected by the Board of Directors of the Coalition for a Healthier Frederick County through a robust assessment process and with considerable community input. The next step in building community health involves bringing together members of the public and community partners to form Local Health Improvement Planning Workgroups, where ideas are turned into action.
Adverse Childhood Experiences in Adolescents
Raising awareness about the impact of toxic stress on children and families and promoting systems that build healthy brains and reduce the effects of toxic stress
Type 2 Diabetes
Broadening awareness of type 2 diabetes amongst community members and the medical community to make it easier to prevent and manage this chronic illness
Mental Health
Helping the community understand that “mental health” is “health,” because mental health disorders are common, treatable, and in many cases, preventable
Frederick County Community Health Needs Assessments
The 2025 Community Health Needs Assessment for Frederick County, MD report is currently in progress. A final report is expected by May 2025. This report is completed in partnership with the Frederick Health system and the Coalition for a Healthier Frederick County.
