Uniting Us All Towards a Healthier Future
Effective collaboration is difficult, especially on a topic as complex as population health. Our Coalition provides structure and accountability for passionate leaders and their organizations to collaborate towards a healthier community.
Frederick County Health Department has designated the Coalition as the Local Health Improvement Coalition for Frederick County.
Our Mission is to improve wellness and resiliency to equitably impact the lifelong health of all Frederick County residents.
We achieve this mission by coordinating local partners to:
Identify areas of need based on local health data for the Community Health Needs Assessment (CHNA)
Respond to the CHNA with a Local Health Improvement Plan (LHIP), which identifies priorities and creates an actionable plan
Organize those priorities into workgroups constructed of local partners
Rally local organizations to support the workgroups and coordinate their ongoing work to keep progress moving forward
Our Pledge to Health Equity
Health equity means making sure everyone in Frederick County has a fair chance at being healthy, no matter where they live, how much money they make, or what background they come from. Right now, we know not all neighborhoods have the same access to safe housing, good jobs, reliable transportation, or quality healthcare.
These differences show up as gaps in health outcomes. By focusing on health equity, we can close those gaps so every resident, from Brunswick to Thurmont to downtown Frederick, has the same opportunity to thrive.
Health Equity Framework
Our health is shaped by whether families have access to safe housing, healthy food, reliable transportation, and fair opportunities. It should not depend on where you live, how much you earn, or the language you speak.
The Health Equity Framework is Healthier Frederick’s response to the barriers, challenges, and disparities still faced by many Frederick County residents. Grounded in shared priorities and shaped by local voices, it lays out principles and actions to remove barriers and expand opportunities. Together, we can use this roadmap to build stronger, healthier communities across the county.
The Health Equity Brochure and Rack Card share an overview of how Healthier Frederick is working to support the conditions that help all people live a healthy life. They highlight our purpose for advancing health equity, commitment to strong and lasting partnerships, and our short-, medium-, and long-term goals.

Current Priorities
The 2025 Community Health Needs Assessment (CHNA), our most community-informed yet, identified five priority areas to guide the Local Health Improvement Process (LHIP) Workgroups’ efforts over the 2025-2028 LHIP cycle. Local changemakers and community voices will unite in these four Workgroups to transform shared goals into real, lasting impact.
Lack of Access to Affordable Healthy Food
Addressing barriers to affordable, nutritious food and advancing community-driven solutions that promote equitable food access for all.
Institutional and Systemic Racism
Addressing the shortage of affordable housing by elevating community voices to support inclusive development and collaborative solutions that ensure safe, stable homes for all.
Mental Health: Lack of Awareness of Accessibility of Resources & Stigma
Promoting mental well-being by increasing access to and awareness of resources, reducing stigma, and strengthening systems of support across the community.
Inadequate Affordable
Housing Supply
Challenging institutional and systemic racism by supporting partnerships, practices, and policies that advance racial equity and social justice.

We are a nonprofit organization that fulfills Frederick County’s requirement for a local health improvement coalition. We manage the county's Community Health Needs Assessment (CHNA) and Local Health Improvement Process (LHIP) in close collaboration with the Frederick County Health Department, Frederick Health, and many community partners.
