Uniting us all towards a healthier future
Effective collaboration is difficult, especially on a topic as complex as population health. Our Coalition provides structure and accountability for passionate leaders and their organizations to collaborate towards a healthier community.
Frederick County Health Department has designated the Coalition as the Local Health Improvement Coalition for Frederick County.
Our Mission is to improve wellness and resiliency to equitably impact the lifelong health of all Frederick County residents.
We achieve this mission by coordinating local partners to:
Identify areas of need based on local health data for the Community Health Needs Assessment (CHNA)
Respond to the CHNA with a Local Health Improvement Plan (LHIP), which identifies priorities and creates an actionable plan
Organize those priorities into workgroups constructed of local partners
Rally local organizations to support the workgroups and coordinate their ongoing work to keep progress moving forward
Current Priorities
These priorities were selected through a robust assessment process with substantial community input within the Community Health Needs Assessment (CHNA) process. Now, as part of our Local Health Improvement Plan (LHIP), health partners collaborate with members of our community in one of our three Health Improvement Planning Workgroups and turn ideas into action.
Adverse Childhood Experiences in Adolescents
Raising awareness about the impact of toxic stress on children and families and promoting systems that build healthy brains and reduce the effects of toxic stress
Type 2 Diabetes
Broadening awareness of type 2 diabetes amongst community members and the medical community to make it easier to prevent and manage this chronic illness
Mental Health
Helping the community understand that “mental health” is “health,” because mental health disorders are common, treatable, and in many cases, preventable
Helping 200+ organizations work as a team
We are a nonprofit organization that fulfills Frederick County’s requirement for a healthcare coalition. We manage the county's Community Health Needs Assessment (CHNA) and Local Health Improvement Process (LHIP) in close collaboration with the Frederick County Health Department, Frederick Health, and many community partners.
Our partners include charitable and religious groups, business and corporate entities, state and local government organizations, healthcare providers, private and non-profit organizations, coalitions and citizen groups, and individuals. More than 200 partners work with us to have a collective impact on health in Frederick County.
The health and wellbeing of all of our residents is the foundation for a thriving and resilient community and economy. We provide a pathway for local action and public engagement to advance the health of Frederick County residents.
