Grievance Procedures and General Complaint Form
The General Complaint Form may be used to anonymously report a complaint against Healthier Frederick’s Executive Director, staff member(s), Workgroup volunteer(s), and/or board member(s) and is used in conjunction with our Grievance Procedures. It is not intended for whistleblowing purposes.
Whistleblower Policy and Whistleblower Form
Healthier Frederick is committed to maintaining integrity, accountability, and transparency in all of its operations. The Whistleblower Form is intended to provide a way for individuals to report concerns about suspected fraud or financial misconduct, misuse of organizational resources, retaliation, and/or other ethical problems.
If your concern involves discrimination, bullying, sexual harassment, harassment, or unfair treatment, please refer to Healthier Frederick's Grievance Procedures and General Complaint Form above.
